Waiver

Acknowledgment and Assumption of Risk

I, the undersigned, acknowledge that participation in martial arts training at Hierba Mala Brazilian Jiu-Jitsu involves inherent risks, including but not limited to physical injury, disability, and, in rare cases, death. I understand that martial arts is a contact activity and that injuries may result from my participation, including those caused by falls, contact with other participants, equipment failure, or the conditions of the facility.I voluntarily assume all risks associated with my participation in martial arts training and acknowledge that Hierba Mala Brazilian Jiu-Jitsu, its instructors, staff, affiliates, and representatives are not responsible for any injuries, damages, or losses that I may sustain.

Release of Liability

I hereby release, discharge, and agree not to sue Hierba Mala Brazilian Jiu-Jitsu, its owners, instructors, employees, agents, or representatives from any and all claims, liabilities, demands, or causes of action that may arise from my participation in activities at the school, whether caused by negligence or otherwise. I understand that this release includes any claims based on the actions, omissions, or negligence of Hierba Mala Brazilian Jiu-Jitsu, its employees, volunteers, and representatives.

Medical Authorization

In the event of an emergency, I authorize Hierba Mala Brazilian Jiu-Jitsu and its representatives to seek medical attention on my behalf if I am unable to do so. I understand that I am responsible for any medical expenses incurred as a result of my participation in martial arts training.

Rules and Code of Conduct

I agree to abide by all rules and regulations set forth by Hierba Mala Brazilian Jiu-Jitsu. I acknowledge that failure to follow instructions or engage in unsafe behavior may result in my dismissal from the program without refund.

Indemnification

I agree to indemnify and hold harmless Hierba Mala Brazilian Jiu-Jitsu, its instructors, employees, and affiliates from any claims, damages, or costs incurred due to my participation in martial arts activities.

Photo and Video Release

I grant permission for Hierba Mala Brazilian Jiu-Jitsu to use photographs or videos of me for promotional, instructional, or advertising purposes without compensation. If I do not wish to be photographed or recorded, I will notify the school in writing.

Acknowledgment of Terms and Conditions

I have read and understood the Terms and Conditions and Privacy Policy of Hierba Mala Brazilian Jiu-Jitsu and agree to be bound by them.
